Sub-Acute Care

All Saints Sub-Acute & Skilled Nursing is pleased to provide a sub-acute care program for residents recently released from the hospital. Our main goal is to reduce the risk of re-admission to the hospital as well as weaning our patients off of tracheostomy and ventilators to get them to a lower level of care. We wish to promote quality of life to each patient.

We seek to help our patients in the recuperation process while helping them achieve their maximum capabilities to return home as quickly as possible.

Sub-acute care is for residents who are medically fragile and require special services. These services include:

  • Inhalation therapy
  • Tracheotomy care
  • Intravenous tube feeding
  • Complex wound management
  • Brain injury
  • Spinal injuries

Severe injury often affects the whole person – body, mind and spirit. Our nurses are available 24 hours a day. We also have our therapists, dietitians, and case managers available to fit each individuals schedule.
